Preparation: The water supply shut off valves must be turned off- see article How to Turn Off the Water Supply to the Faucet.

For disassembly, follow the number of the parts in order starting with the screw (#1).

For re-assembly, rotate the stem extension (#2) to the "off" position (clockwise for hot, counter clockwise for cold) until it stops and thread the screw (#1) back in .
